Tony’s PTSD Case
Tony is a 30-year-old White male who was presented for treatment approximately six months after he was discharged from the military for a service-connected disability. Tony was serving in Iraq when an improvised explosive device hit their convoy. The event led to multiple casualties, and Tom was lucky to escape with a severe injury to his arm. However, the injury meant he could no longer continue serving, and was discharged based on a service-connected disability. After discharge, Tony underwent six months of physical rehabilitation, and it was during this rehabilitation, that psychological distress symptoms began manifesting. This manifestation prompted the hospital to refer Tony to the mental health center.
At the center, the diagnosis shows that Tony met the DSM 5 criteria for PTSD and alcohol use disorder as a co-occurring disorder. According to Reichenberg and Seligman (2016), one is diagnosed with PTSD following exposure to a traumatic event and if they demonstrate one or more of various intrusive symptoms and arousal responses related to PTSD for at least one month. Tony meets this criterion, where they report manifesting most of these symptoms and arousal responses for the last three months. For example, he reported having recurrent frightening dreams and intrusive memories of the event. In addition, Tony reported experiencing general anxiety and panic attacks. Further, while he was previously a lively and sociable person, he noted that he had become detached from close friends and family and was disinterested in activities that he used to enjoy. He was also easily irritable and would lash at people close to him. Further assessment revealed that Tony also had alcohol use disorder as a co-occurring disorder. For example, he reported having turned to alcohol to numb his emotions and considered it his remedy to divert him from the intrusive memories of the event. Therefore, Tony meets the DSM 5 criteria for PTSD and alcohol use disorder.
