Blepharitis Case study
Mr Williams is a 55-year-old car mechanic who has been complaining of bilateral sore eyes and redness for the past two months. He lives in a crowded two-bedroom flat with his wife and three teenage daughters. He is a heavy smoker and also drinks daily with work colleagues in the local pub. He is diabetic, for which he takes metformin 500mg twice daily. He does not eat healthily. He has financial worries and is in rent arrears He has been referred to the eye clinic by his GP, as his eyes have not responded to chloramphenicol 0.5% eye drops, 1 drop 4-hourly, for the past five days. His symptoms feel much worse. His right vision is reduced and he has a constant ‘foreign body sensation’ in the right eye. His eyes are sticky on waking up. He also complains of a dry burning sensation in both eyes.
The ophthalmologist assesses his eyes and notes that he has severe crust on the lid margins. A diagnosis of Staphylococcal blepharitis is made. There is also a small ulcer near his right cornea. He is very worried about his sight and also about taking time off work.
