Fiona is a 32-year-old woman with a diagnosis of depression. Her symptoms are that she is low in mood with anhedonia (loss of ability to experience pleasure). She says that she feels really ‘slowed up’. Fiona tends to spend most of the day sitting on her sofa with the TV on but is not really watching it. She says she has difficulty concentrating and her memory is poor.
At night her sleep is somewhat disturbed, and she wakes quite frequently. As a result, she sleeps on and off throughout the day. Fiona has suicidal thoughts at times but does not have any plans as to how she would actually end her life. She says her thought processes cannot really focus on that sort of detail.
Fiona has been prescribed citalopram 20mg once daily.
After six weeks, Fiona has noticed some improvement in her sleep but her mood has not lifted.
After a further four weeks, Fiona is still not noticing any improvement in her mood. You discuss her case with your consultant, who suggests switching to venlafaxine.
