Introduction to Assignment:
Demographic and Epidemiological Data help to describe the health and wellness of communities and population groups. These data help to identify factors that contribute to disease, detect community/population health problems, and define beneficial health promotion and disease prevention measures.
According to McFarlane and Gilroy (2019) “Demography…is the statistical study of human populations with reference to size and density, distribution, and vital statistics. Demographic statistics provide information about significant characteristics of a population that influence community needs and the delivery of health care” (p. 37).
Epidemiology “investigates the distribution or the patterns of health events in populations and the determinants or the factors that influence those patterns” (Messias & Adams, 2017, p. 148).
In this portion of the Community Assessment, students will look at key demographic and epidemiological data for their selected County (This is the County you selected in Module 5)
Directions for the Assignment:
In the template below, provide the indicated demographic and epidemiological data for your county, for the categories listed in each table.
Analyze the findings from the demographic and epidemiological data in the Data Analysis table. Analysis should be objective and supported with specific findings from the data.
Provide citations for information from published sources using APA format for citations/references.
Use third person view for the summary to maintain objectivity.

Demographic Data can be located at United States Census Bureau (United States Census Bureau, n.d.)
In the search box at the above site, type in your county and state, then click search. This will take you to the demographic data for your county.
To locate data for the United States, type United States in the search box then click search.
Epidemiological Data
Epidemiological data can be found at County Health Rankings and Roadmaps (County Health Rankings & Roadmaps, 2020).
In the search box located under the heading How Healthy is Your Community enter your County then click search.
Review the data for a) You’re County, b) Top U.S. Performers, and c) Your State. (Note: Top U.S. Performers are Counties that are ranked in the top 10% for that Factor)
Data Analysis

Write a 2-paragraph summary of the findings from the demographic and epidemiological data for the county. Apply critical thinking to identify key findings from the data. The summary should be objective and supported by specific data.
Potential Health Promotion Topics
Population-focused Health Promotion activities aim to empower communities and population groups to choose healthy behaviors, engage in wellness activities, and reduce risk factors for disease and chronic illness.
Health Promotion activities begin with identifying health issues or risk factors of concern for communities and population groups, based on demographic and epidemiological data and community health assessment.
