Describe the foundations of mental health nursing.
The foundations of mental health nursing have evolved. This competency was achieved by a discussion post related to our perceptions about our current mental health system. I believe that our current mental health system is severely broken. The social stigma severely affects the economy, as well as people who don’t want to be classified as having to be “crazy or psycho.”
One thing I learned was about St. Elizabeth’s. I was unaware of the deinstitutionalization of this facility back in the 1960s. This new knowledge will impact me as a future nurse because I will relate and focus on how mental health has evolved. Although it may not be going in the right direction, hopefully, mental health issues will be something everyone is comfortable with.
Explain current legal issues and social concerns in mental healthcare.
In Module 3, we discussed legal and social issues in mental healthcare. Ethics is a major topic that is discussed in healthcare. We learned about the different ethical principles, clients’ rights, HIPPA, and restraints in this module. One thing that I learned was the involuntary admissions are limited to up to 60 days.
However, can be extended if they are a harm to themself or others (Morgan, 2021). This new knowledge will be useful as a future nurse because clients have specific rights when admitted to a facility. I will apply this knowledge to my practice because each client is entitled to their legal rights.

In Module 4, we learned about neurological disorders and conditions that can affect memory or change a level of functioning. In this module, we learned about the differences between dementia and delirium. Dementia is a mental disorder that involves a progressive functional decline in cognitive status.
Delirium is a condition that has an acute onset change in cognition that commonly occurs from medical, surgical, or neurological conditions (Morgan, 2021). I will apply this new knowledge by recognizing the signs and symptoms that my client presents if it’s an acute change because this could have occurred due to a medical condition. Often, we treat these clients with medication or a consultation with psychiatry. However, I will use these skills and apply them to my medical practice.
Evaluate nursing care strategies for clients with certain mental health/mood/personality and eating disorders.
In Module 6 and 7, we discussed strategies with mood, personality, and eating disorders clients. In module 6, bipolar and depressive disorders were the main topics that were focused on. The most important thing was to ensure safety in these patients. They are at high risk for suicide.
So, we want to ensure safety to our patients by suicide assessment, promoting a safe environment, and providing one-to-one supervision. In module 7, we focused on personality disorders and the fact that they amplify emotional dysregulation (Morgan, 2021). There are 10 personality disorders that are grouped into clusters A, B, and C due to similar behavior patterns and personality traits.
One thing I learned in Module 6 is that clients who are taking antidepressants are at high risk for suicide when they “regain energy.” This was something I was very unaware of and I will apply this to my nursing practice.
One thing I learned in Module 7 is that for borderline personality disorder they are at high risk for self-mutilation. For these specific disorders, some nursing interventions would include making aware of behaviors that are acceptable and those that are not, use the least restrictive method, and recognize manipulative behaviors. I will apply these interventions when I have contact with a client with this disorder. It will help me provide the best care to the client as well as meet their needs.

Identify factors that impact mental health in special populations.
In Module 8 we focused on clients with mental health and stress-related disorders. In module 10, we focused on mental health in special populations. In module 8, anxiety and obsessive-compulsive disorders were the main areas focused on. We learned that normal anxiety is a healthy reaction. Our body reacts to both anxiety and fear. Anxiety is an emotional process that invades personality and feelings of self-esteem and fear is a cognitive process (Morgan, 2021).
One thing I learned is how to use nursing interventions for someone who is experiencing anxiety. For example, want to minimize external stimuli, use clear and simple statements, use low-pitched voice, speak slowly, present reality, and reassure the client of safety and security. I will apply these interventions as a future nurse when I have a client who is experiencing anxiety. This is something that is very common in health care.
I completed this competency by writing a case study about anxiety and OCD. I provided examples of anxiety and OCD and the knowledge of different types of therapeutic communication skills. Also, interventions and rationales would help Lauren deal with her disorder.
In Module 10, the areas of focus were child abuse, intimate partner violence, older adult abuse, and end of life. Abuse is an important topic to focus on because it is something that occurs. Often, as healthcare professionals, we are hesitant to report something. We don’t want to make someone else mad or have false reports. One thing I found interesting was that Erik Erikson described the worst sin someone could commit as the mutilation of a child’s spirit (Morgan, 2021).
It saddens me when I hear about child abuse and how someone could do something so harmful to a child who is unable to defend themself. As a future nurse, I will use correct judgment skills and understand the signs of physical, emotional, and sexual abuse. I will not be hesitant to report (based on my objective data) to the correct officials.

This competency mainly covered the biology of the mind/brain, drugs, and legal-ethical issues in mental healthcare. One major concept I learned that even people that are involuntarily admitted, have the right to refuse ordered medication unless their safety or the safety of other’s is compromised, or it is court ordered (Morgan, 2021).

Some legal issues in mental healthcare are elder abuse, the use of restrains and seclusion, and privacy or protected information “HIPPA” laws. The elderly, demented individuals are at high risk of abuse by staff. Restraints must be used only when safety is compromised or when consent has been signed for use during a procedure.
Informed consent must be achieved in order to do any surgery. Social issues revolve around the fact that many mentally ill people struggle finding/keeping steady jobs. They are often homeless and continue in a downward spiral with crime and drugs (Morgan, 2021).

When caring for cognitively ill clients, the goal is to maintain safety and dignity, and decelerate the process. When working with intoxicated or psychotic clients, protect your own safety first by assuring that you have the appropriate security in place.
Keep sentences short and simple. Set boundaries. Observe signs of withdrawal and alert provider immediately for medication to relieve symptoms. Encourage expression of feelings and thoughts by MH clients. Provide emotional support and encourage activities to maintain/regain independence (Morgan, 2021).

You must set boundaries on behavior and ensure clients feel safe. Manic clients must be provided frequent rest periods, finger foods, and a physical outlet. Depressive clients always must be monitored for self-harm. For anorexic/bulimic clients, stay with them for an hour after eating to ensure they do not vomit. Start with small meals at first to avoid overwhelming these clients (Morgan, 2021).

It is important to encourage expression of anger or grief for stress related issues. Help client gain independency and identify resources for support (Morgan, 2021).
Identify factors that impact mental health in special populations.
A history of abuse (violent, sexual, psychological) can severely affect the mental health of the victim. Age is a factor here. Children are vulnerable and often abused by family especially low-income and intoxicated parents. The elderly are vulnerable and often abused or neglected. Being female also increased risk for sexual abuse. Being disabled also makes you more vulnerable. Being poor or homeless also increases risk of abuse (Morgan, 2021).

I achieved the skill of communication by studying the therapeutic vs non-therapeutic methods of interviewing. For example, restating helps the client feel understood and heard while asking why provokes defensiveness (Morgan, 2021).

This new knowledge will help me be a better nurse. I will know the signs of mental health illness and how to properly care for these individuals.
